Study Carrel Request Form

UWF faculty and graduate students may request a study carrel for a semester. Please complete and submit this form at least one week prior to the beginning of the semester you are requesting a carrel. Carrel keys may be picked up at the Circulation Desk.

NOTE: If you apply for a carrel before the semester starts, your carrel key may be picked up during the first week of classes. If you apply after the semester begins, you will be able to pick up your carrel key after two working days. You will receive an e-mail notifying you that your carrel key is available.

By submitting this form, you agree to the following terms:

1. The occupant is responsible for the study carrel key. A $20.00 replacement charge will be assessed if a key is lost or not returned. Also, a $5.00 replacement charge will be assessed if the tag is damaged or not returned.
2. Carrel occupants are responsible for checking out library materials used in the carrels. Journals or reference books may not be left in the carrels. Carrels will be checked regularly for library materials and all uncharged library materials will be returned to the stacks.
3. Large library-owned lounge chairs are not to be brought into the carrel.
4. For safety and security reasons, the glass on the carrel door may not be covered.
5. Aside from normal wear and tear, the carrel occupant is responsible for any damage done to the carrel.
6. The Library is NOT responsible for any personal property left in the carrel.
7. Lights and power to the carrels will be turned off when the library is closed. The Library is not responsible for data lost due to power failures or normal closing procedures.
